Membership in the Medical Staff Organization (MSO)

Only members of the Medical Staff Organization can admit patients to the hospital. However, doctors who are not members of the MSO can refer patients for out-patient diagnostic examinations and therapeutic procedures.

Please read the requirements below and fill out the necessary forms to apply for membership in our Medical Staff Organization. St. Luke's Medical Center is actively recruiting members for the following specialties:

Infectious Diseases - Adult
Genetics
Thoracic Surgery

Requirements for Affiliates/Consultants
  1. Letter of Application (addressed to Dr. Joven R. Cuanang, SVP for Medical Affairs and Medical Director)
  2. Curriculum Vitae
  3. List of two (2) references which should include the Training Officer during the Residency/Fellowship Training.  Please include their complete addresses & contact numbers.
  4. Pictures: 1x1 and 2x2 (1 each)
  5. Medical Board Certificate
  6. Residency Training Certificate
  7. Fellowship Training Certificate (if applicable
  8. Specialty Board Certificate
  9. Photocopy of Philippine Medical Association (PMA)/ Philippine Dental Association (PDA) Membership ID
  10. Photocopy of Professional Regulation Commission ID (updated PRC Card)
  11. Photocopy of Professional Tax Receipt (PTR)
  12. Photocopy of Life Support Certification (Advance or Basic-whichever is applicable) (Please see note below)
Additional Requirement for Orthopedic Applicants

          13.  Fellowship Training - POA Certificate

Please note:

  • Incomplete papers will not be accepted or processed.
  • While papers are still being processed, a Special Privilege (SP) Permit can be issued on a case- to-case basis to applicants who may have patients for admission.
  • Members of the following specialties are required to submit certificate of completion of:

    Endocrine, Diabetes & Metabolism

    Basic Life Support (BLS)
    Allied Medical Professionals
    Dentistry
    Dermatology
    ENT-HNS
    Neurology
    Neurosurgery
    Nuclear Medicine
    Obstetrics & Gynecology
    Ophthalmology
    Orthopedics
    Pathology
    Psychiatry
    Radiology
    Rehabilitation Medicine

    Pediatric Advanced Life Support (PALS)/Neonatal Resuscitation Program (NRP)

    Note: Certification is valid for two (2) years only
